Abstract
In this
quantitative survey research study, a structured questionnaire was designed and
administered to 120 respondents to investigate the collaboration between
midwives and obstetricians in providing maternal care at the Federal Medical
Center Yenagoa. The study aimed to assess the existing level of collaboration,
identify challenges, and explore potential improvements in this critical aspect
of healthcare. The data collected through the questionnaire were processed and
analyzed using SPSS27. To test the hypotheses formulated, a t-test was applied,
allowing for a statistical assessment of the research questions. The study
found that there is room for improvement in the collaboration between midwives
and obstetricians at the Federal Medical Center Yenagoa, highlighting the need
for targeted interventions to enhance teamwork and communication. The results
revealed that respondents generally acknowledged the importance of
collaboration between midwives and obstetricians, with a significant portion
expressing agreement (58.6%) and uncertainty (15.4%) regarding its
effectiveness. Challenges related to communication and resource constraints
were also identified as hindrances to effective collaboration. In conclusion,
the study underscores the significance of fostering effective collaboration
between midwives and obstetricians to ensure safe and positive birth
experiences. The identified challenges in communication and resource
availability call for targeted interventions and improvements in these areas.
Based on the findings, the study recommends the implementation of communication
enhancement programs, resource allocation, interprofessional education,
standardized protocols, regular interprofessional meetings, feedback
mechanisms, a patient-centered care approach, and continued research and
evaluation of interprofessional collaboration in maternal care. These
recommendations collectively aim to strengthen the collaboration between
midwives and obstetricians at the Federal Medical Center Yenagoa and similar
healthcare facilities, ultimately leading to improved maternal care and better
health outcomes for both mothers and newborns.

INTRODUCTION
1.1
Background to the Study
Childbirth
is a transformative experience that holds profound significance for expectant
mothers and their families (Aquino et al., 2016). In Nigeria, the Federal
Medical Center Yenagoa stands as a pivotal institution in providing healthcare
and support to women during childbirth. The quality of care and the overall
experience of childbirth are deeply influenced by the collaboration between
midwives and obstetricians in this healthcare facility (Stahl & Agricola,
2021). Midwives are essential in promoting natural childbirth, while
obstetricians specialize in managing high-risk pregnancies and complications
(Smith, 2015). This collaboration is fundamental to ensuring safe and positive
birth experiences at the Federal Medical Center Yenagoa (Smith, 2015).
Midwives
are essential figures in promoting natural childbirth and providing
comprehensive care to expectant mothers. Their role extends far beyond merely
assisting with the delivery; they offer guidance, emotional support, and
education throughout the childbirth process (Aquino et al., 2016). Midwives
possess a unique skill set and expertise in low-risk pregnancies, ensuring that
women can have a natural, empowering, and less medicalized childbirth
experience (Stahl, 2016). They focus on the emotional and psychological
well-being of mothers, understanding that the journey of childbirth goes beyond
the physical aspects (Scheerhagen et al., 2015).
At
the Federal Medical Center Yenagoa, midwives play a central role in supporting
women who aim to have a natural and uncomplicated childbirth. They work with
expectant mothers throughout their pregnancy, providing prenatal care,
education, and emotional support (Warmelink et al., 2017). This collaborative
approach between midwives and mothers helps create a sense of trust and
security, which is essential for a positive birth experience (Sandall et al.,
2016). By fostering a connection between midwives and expectant mothers, the
healthcare facility emphasizes the importance of patient-centered care, a core
principle in ensuring a positive childbirth experience (Smith, 2015).
On
the other hand, obstetricians are specialists in managing high-risk pregnancies
and complications (Aquino et al., 2016). While midwives focus on low-risk,
uncomplicated deliveries, obstetricians are equipped to handle more complex
cases (Smith, 2015). Their expertise is crucial for identifying and addressing
potential complications, ensuring the safety of both the mother and the baby
(Aquino et al., 2016). In high-risk pregnancies, the collaboration between
midwives and obstetricians becomes even more critical (Smith, 2015).
The
interprofessional collaboration between midwives and obstetricians at the
Federal Medical Center Yenagoa is a testament to the "team approach"
recommended by the World Health Organization (WHO) to enhance the quality of
maternal and newborn care (WHO, 2020). This approach recognizes that the skills
and expertise of different healthcare professionals are complementary and
necessary for providing comprehensive care. The collaboration fosters an
environment where trust and mutual respect are paramount, ensuring that the
focus remains on the well-being of the mother and the baby (Aquino et al.,
2016).
The
collaboration between midwives and obstetricians also aligns with global
standards, such as the WHO's Framework for Action on Interprofessional
Education & Collaborative Practice (WHO, 2020). This framework emphasizes
the importance of teamwork among healthcare professionals to improve patient
outcomes and enhance the quality of care. At the Federal Medical Center
Yenagoa, this collaboration is not just a bureaucratic requirement but a
practical solution that saves lives and ensures the best possible childbirth
experience for women (Smith, 2015).
Collaboration,
however, comes with its unique challenges and nuances (Reeves et al., 2017). In
the context of maternal care in Nigeria, the collaboration between midwives and
obstetricians faces various barriers, including issues of hierarchy, communication,
and resource constraints (Reiger, 2018). Overcoming these challenges is crucial
for the successful implementation of interprofessional collaboration (Stahl,
2021).
One
of the challenges is related to the existing hierarchy in healthcare settings
(Reiger, 2018). Obstetricians often hold higher positions and more authority,
which can lead to a power imbalance within the collaborative team (Reiger,
2018). This hierarchy can sometimes hinder effective communication and the
sharing of responsibilities (Weller et al., 2014). Addressing this issue
requires a shift in the cultural dynamics within healthcare institutions,
emphasizing that each team member's expertise is equally valuable (Reiger,
2018).
Effective
communication is another critical aspect of collaboration (Reeves et al.,
2017). Miscommunication or a lack of clear communication channels can lead to
misunderstandings and, in some cases, compromised patient care (Reeves et al.,
2017). Establishing standardized protocols for interprofessional communication
and encouraging open dialogue between midwives and obstetricians can help
overcome this challenge (Reeves et al., 2017).
Resource
constraints, such as limited medical supplies and staff shortages, can further
complicate the collaborative process (Reeves et al., 2017). Adequate resource
allocation is necessary to ensure that both midwives and obstetricians have the
tools and support they need to provide high-quality care (Reeves et al., 2017).
Advocating for better resource allocation within the healthcare system is a
collective effort that can significantly enhance the quality of maternal care
(Stahl & Agricola, 2021).
In
conclusion, the collaboration between midwives and obstetricians at the Federal
Medical Center Yenagoa is fundamental to ensuring safe and positive birth
experiences for expectant mothers and their families in Nigeria (Aquino et al.,
2016). Midwives play a vital role in promoting natural childbirth and providing
comprehensive care, while obstetricians specialize in managing high-risk
pregnancies and complications (Smith, 2015). This interprofessional
collaboration aligns with global standards and recommendations, emphasizing the
importance of teamwork to enhance the quality of maternal and newborn care
(WHO, 2020).
While
challenges related to hierarchy, communication, and resource constraints exist,
they can be addressed through a commitment to cultural change, standardized
protocols, and advocating for better resource allocation (Reiger, 2018; Reeves
et al., 2017; Stahl & Agricola, 2021). The Federal Medical Center Yenagoa's
dedication to interprofessional collaboration reflects its commitment to
providing the best possible care to women during one of life's most significant
moments – childbirth (Stahl & Agricola, 2021). This commitment has a
profound impact on the lives of expectant mothers and their families, ensuring
that childbirth in Nigeria becomes a transformative and positive experience
(Aquino et al., 2016).
1.2
Statement of Problem
The
statement of the problem is a critical component of any research study and
serves as the foundation upon which the entire investigation is built. In the
context of maternal care and interprofessional collaboration at the Federal
Medical Center Yenagoa, Nigeria, the statement of the problem outlines the
specific issues and challenges that necessitate further research and attention.
Maternal
care is a vital aspect of healthcare, and the experience of childbirth
significantly impacts the lives of women and their families. Nigeria, like many
countries, faces challenges in providing quality maternal care services, often
resulting in adverse maternal and neonatal outcomes (Tunçalp et al., 2015).
Inadequate access to quality healthcare services, limited resources, and
variations in healthcare provider competencies are some of the factors
contributing to these challenges (Stahl & Agricola, 2021).
The
statement of the problem for this research study emphasizes the need to address
the complex interplay between midwives and obstetricians in the delivery of
maternal care. While both professionals play essential roles, collaboration
between them is not always seamless, and this may hinder the provision of
optimal care (Reiger & Lane, 2019).
Issues
such as hierarchies and power differentials within healthcare teams,
communication gaps, and resource constraints impact the collaborative practices
between midwives and obstetricians (Reiger & Lane, 2019; Reeves et al.,
2017). These challenges can affect the quality of maternal care services, lead
to misunderstandings and mismanagement of high-risk pregnancies, and,
ultimately, compromise the safety and satisfaction of expectant mothers
(Reiger, 2018).
The
statement of the problem recognizes that improving interprofessional
collaboration between midwives and obstetricians is essential to address these
issues and enhance the overall quality of maternal care at the Federal Medical
Center Yenagoa. By identifying the existing challenges, this research aims to
contribute to the development of solutions that can lead to better maternal
care, safer childbirth experiences, and positive outcomes for both women and
their families.
