INVESTIGATION OF SOCIOCULTURAL RISK FACTORS FOR MATERNAL MORTALITY AND ITS EFFECT ON PREGNANT WOMEN ATTENDING ANTENATAL CLINIC IN FAITH MEDIPLEX HOSPITAL

INTRODUCTION

1.1 Background to the Study

Maternal mortality remains a significant public health concern worldwide, particularly in developing countries where access to adequate healthcare services is often limited (Miltenburg et al., 2017). Despite global efforts to reduce maternal mortality rates, many women still die from pregnancy-related complications, making it a critical issue that demands attention. In Nigeria, maternal mortality rates are notably high, with sociocultural factors playing a significant role in exacerbating this problem (Fagbamigbe & Idemudia, 2019). Understanding these sociocultural risk factors is essential for developing effective interventions to address maternal mortality and improve maternal health outcomes.

One of the key sociocultural factors contributing to high maternal mortality rates in Nigeria is the prevalence of early age marriages (Qamar et al., 2020). Young girls who get married early often lack access to proper maternal healthcare services, leading to increased risks during pregnancy and childbirth. Additionally, the practice of early age marriages may result in social and economic dependence on spouses, limiting women's autonomy and decision-making power regarding their own health (Kaur & Gupta, 2018). Addressing the root causes of early age marriages and promoting education and empowerment among young girls are crucial steps in reducing maternal mortality rates in Nigeria.

Another sociocultural determinant of maternal mortality in Nigeria is the acceptance and perpetuation of certain cultural practices, such as the preference for home births over institutional deliveries (Evans, 2021). Despite efforts to promote facility-based deliveries, many women in rural areas still opt for traditional home births due to cultural beliefs and practices (Choudhary et al., 2017). This preference for home births increases the risk of complications during delivery, as women may not have access to skilled birth attendants or emergency obstetric care when needed (Sk et al., 2019). Educating communities about the importance of skilled attendance at birth and addressing misconceptions surrounding home births are essential strategies for reducing maternal mortality in Nigeria.

In addition to cultural practices, social factors such as poverty and limited access to healthcare services also contribute to high maternal mortality rates in Nigeria (Fawole & Adeoye, 2021). Women from low-income households often face barriers to accessing maternal healthcare services, including transportation costs, lack of health insurance, and inadequate infrastructure (Naz & Ahmad, 2022). Furthermore, stigma and discrimination against marginalized groups, such as women from rural areas or ethnic minorities, may prevent them from seeking timely and appropriate care (Naz et al., 2022). Addressing socioeconomic disparities and improving access to quality healthcare services for all women are essential for reducing maternal mortality rates in Nigeria.

Moreover, the role of healthcare providers and health systems in addressing maternal mortality cannot be overstated (Thaddeus & Maine, 2022). Inadequate training of healthcare providers, limited availability of essential obstetric supplies, and weak health systems contribute to suboptimal maternal health outcomes in Nigeria (Miltenburg et al., 2017). Strengthening the capacity of healthcare providers, ensuring the availability of essential drugs and equipment, and improving the overall quality of maternal healthcare services are critical interventions for reducing maternal mortality rates in the country (Naz & Chaudhry, 2021).

In essence, addressing maternal mortality in Nigeria requires a multifaceted approach that considers sociocultural, economic, and healthcare system factors. Efforts to improve maternal health outcomes must focus on empowering women, promoting education, challenging harmful cultural practices, and strengthening healthcare systems. By addressing these underlying determinants of maternal mortality, Nigeria can make significant strides towards achieving better maternal health outcomes and reducing the burden of preventable maternal deaths.

1.2 Statement of Problem

Maternal mortality continues to be a pressing public health issue worldwide, particularly in developing countries like Nigeria, where rates remain unacceptably high (Miltenburg et al., 2017). Despite concerted efforts to address this problem, significant gaps persist in understanding the underlying factors contributing to maternal deaths and implementing effective interventions to mitigate them. One of the key gaps is the lack of comprehensive data on the specific causes of maternal mortality, particularly in low-resource settings (Riley et al., 2019). Verbal autopsy methods have been used to monitor progress in reducing maternal mortality, but there is a need for more robust vital registration systems to accurately capture and analyze maternal deaths (Riley et al., 2019). Additionally, while global reports provide estimates of maternal mortality trends, there is limited disaggregated data at the country level, hindering the development of targeted interventions (World Health Organization, 2019).

Furthermore, sociocultural factors play a significant role in maternal mortality, yet there is a lack of in-depth research on their influence and how they intersect with other determinants of health (Evans, 2021). Cultural practices such as early age marriages and preference for home births contribute to increased maternal mortality rates but remain poorly understood in terms of their impact and the underlying mechanisms driving them (Fagbamigbe & Idemudia, 2019; Choudhary et al., 2017). Moreover, while there is recognition of the importance of addressing socioeconomic disparities in maternal healthcare access, there is limited research on specific barriers faced by women from marginalized communities and how these barriers can be effectively addressed (Naz et al., 2022).

Additionally, healthcare system weaknesses exacerbate the problem of maternal mortality, yet there is a lack of comprehensive studies assessing the quality of maternal healthcare services and identifying areas for improvement (Miltenburg et al., 2017). Weak health systems, inadequate training of healthcare providers, and limited availability of essential obstetric supplies hinder the provision of timely and quality care to pregnant women (Naz & Chaudhry, 2021). Addressing these gaps in knowledge and understanding is essential for developing evidence-based interventions that can effectively reduce maternal mortality rates in Nigeria and other low-resource settings.

1.3 Objectives of the Study

The specific objectives of this study were as follows:

1. To identify sociocultural risk factors contributing to maternal mortality among pregnant women attending antenatal clinics at Faith Mediplex Hospital.

2. To assess the effect of sociocultural factors on the utilization of maternal healthcare services by pregnant women.

3. To explore the perceptions and experiences of pregnant women regarding sociocultural influences on maternal health outcomes.

1.4 Research Questions

To achieve the objectives outlined above, this study sought answers to the following research questions:

1. What sociocultural factors contribute to maternal mortality among pregnant women attending antenatal clinics at Faith Mediplex Hospital?

2. How do sociocultural factors influence the utilization of maternal healthcare services by pregnant women?

3. What are the perceptions and experiences of pregnant women regarding sociocultural influences on maternal health outcomes?

1.5 Research Hypotheses

Based on the research questions, the following hypotheses were formulated:

1. There is no significant association between sociocultural factors and maternal mortality among pregnant women attending antenatal clinics at Faith Mediplex Hospital.

2. Sociocultural factors do not significantly impact the utilization of maternal healthcare services by pregnant women.

3. Pregnant women's perceptions and experiences are not significantly influenced by sociocultural factors regarding maternal health outcomes.

1.6 Significance of the Study

The findings of this study carry significant implications for maternal healthcare policy, practice, and research. Understanding the sociocultural risk factors identified in this study and their impact on maternal health outcomes can guide healthcare providers and policymakers in the development of targeted interventions aimed at reducing maternal mortality rates and enhancing the quality of maternal health services. By recognizing the specific cultural beliefs, practices, and social norms that influence women's access to and utilization of maternal healthcare services, interventions can be tailored to address these factors effectively. For example, interventions may include community-based education programs to challenge harmful cultural practices or initiatives to improve access to skilled birth attendants in remote areas where home births are prevalent.

Moreover, the findings of this study contribute to the existing body of knowledge on maternal mortality, particularly within the context of Nigerian healthcare settings. By providing insights into the sociocultural determinants of maternal mortality and their interaction with other factors such as socioeconomic status and healthcare infrastructure, this study adds depth and nuance to our understanding of the complexities surrounding maternal health outcomes in Nigeria. This knowledge can inform future research efforts aimed at exploring additional factors contributing to maternal mortality and evaluating the effectiveness of interventions designed to address these factors.

1.7 Scope of the Study

This study focused on pregnant women attending antenatal clinics at Faith Mediplex Hospital, with a specific emphasis on sociocultural factors influencing maternal mortality and healthcare utilization. The research was conducted within a specified timeframe and geographic location to ensure feasibility and relevance to the chosen setting. Data collection methods included surveys, interviews, and review of relevant literature to provide a comprehensive understanding of the research topic.

1.8 Operational Definition of Terms

To ensure clarity and consistency, the following terms are operationally defined within the context of this study:

Maternal Mortality: The death of a woman during pregnancy, childbirth, or within 42 days after the termination of pregnancy, irrespective of the duration and site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management, but not from accidental or incidental causes.

Sociocultural Factors: Refers to the social and cultural conditions, norms, beliefs, practices, and behaviors within a community or society that influence maternal health outcomes.

Antenatal Clinics: Healthcare facilities or programs specifically designed to provide medical care, education, and support to pregnant women during the course of their pregnancy, typically before childbirth.

Utilization of Maternal Healthcare Services: The extent to which pregnant women access and utilize available maternal health services, including antenatal care, delivery services, and postnatal care.

Perceptions: Pregnant women's subjective understanding, beliefs, attitudes, and interpretations regarding sociocultural influences on maternal health.

Experiences: Pregnant women's firsthand encounters, observations, and interactions related to sociocultural factors and maternal health outcomes.

Healthcare Providers: Individuals or organizations involved in the delivery of medical care and services to pregnant women, including doctors, nurses, midwives, and other healthcare professionals.

Policy Interventions: Strategies, programs, or actions implemented by governments, organizations, or stakeholders to address maternal health issues and improve healthcare delivery for pregnant women.
