Abstract

This study examined the knowledge and practices of standard precautions among healthcare workers in Central Hospital Oleh, Delta State. A cross-sectional survey research design was adopted to assess how well healthcare workers understand and apply standard precautions in their daily clinical routines. A structured questionnaire was developed and administered to a sample of 333 respondents, drawn from various professional categories including doctors, nurses, laboratory technicians, and pharmacists. Data collected were analyzed using the Statistical Package for Social Sciences (SPSS) version 27. Descriptive statistics such as frequencies and percentages were used to summarize responses, while one-sample t-tests were conducted to test the three hypotheses formulated. The findings revealed that the majority of respondents demonstrated a high level of knowledge regarding standard precautions, including hand hygiene, use of personal protective equipment (PPE), and proper waste disposal. The results also showed that most healthcare workers practiced standard precautions regularly, though a few reported occasional lapses. The t-test results indicated significant relationships between knowledge and years of experience, between knowledge and actual practice, and between professional category and compliance with standard precautions. These outcomes suggest that experience, professional role, and training play crucial roles in influencing adherence to standard infection control measures. In conclusion, the study found that healthcare workers in Central Hospital Oleh generally possessed good knowledge and demonstrated favorable practices of standard precautions. However, variations existed across different staff categories and experience levels. The study recommended regular training, better supervision, consistent provision of PPE, and tailored infection control strategies for different professional groups to enhance compliance. The findings provide valuable insight for hospital administrators and policymakers aiming to improve infection prevention and control in healthcare settings.

CHAPTER ONE

INTRODUCTION

1.1 Background to the Study

Healthcare delivery inevitably exposes healthcare workers (HCWs) to a wide range of occupational hazards, including infections transmitted through contact with blood, body fluids, secretions, excretions, and contaminated medical instruments or surfaces. These occupational risks are especially high in clinical settings where health workers frequently come in contact with potentially infectious materials during routine care and emergency procedures (Coppola et al., 2022). To minimize these risks, the implementation of standard precautions has become a global best practice in infection prevention and control.

Standard precautions are a set of evidence-based infection control practices designed to reduce the risk of transmission of microorganisms in healthcare settings. They are universally applicable to all patients, regardless of their diagnosis or presumed infection status, and serve as the primary strategy to prevent healthcare-associated infections (Siegel et al., 2023). These precautions include hand hygiene, use of personal protective equipment (PPE), respiratory hygiene and cough etiquette, safe injection practices, environmental cleaning, and the appropriate handling of contaminated medical equipment and linens.

Hand hygiene remains the cornerstone of standard precautions and is recognized as the single most effective method for preventing the spread of infections (Pittet, 2022). However, compliance among HCWs is often suboptimal due to various factors, including insufficient knowledge, high patient load, inadequate infrastructure, and cultural practices (Vaz et al., 2022). Similarly, the consistent use of PPE such as gloves, masks, and gowns, though essential for protection, is often compromised by lack of availability, discomfort, or time constraints (Reda et al., 2022).

Despite the well-established benefits of standard precautions, compliance among HCWs in many developing countries, including Nigeria, remains a significant challenge. Studies have found that poor adherence to standard precautions contributes to the transmission of infections, not only among patients but also among healthcare personnel (Arinze-Onyia et al., 2022). In Nigeria, the healthcare system faces numerous constraints such as limited funding, poor infrastructure, and inadequate policy implementation, which hinder effective infection prevention and control (Tobin et al., 2023).

The burden of healthcare-associated infections (HAIs) in low-resource settings is disproportionately high. Many Nigerian hospitals, including secondary facilities like Central Hospital Oleh in Delta State, face chronic shortages of gloves, disinfectants, running water, and waste disposal systems—factors that impede the routine implementation of standard precautions (Nwoga et al., 2023). Moreover, some HCWs operate in environments where infection control training is not conducted regularly, resulting in knowledge gaps and suboptimal practice (Johnson et al., 2022).

The occupational risks for health workers are further compounded by their exposure to infectious diseases such as hepatitis B and C, HIV/AIDS, and tuberculosis, many of which are prevalent in the Nigerian population. The high rate of hepatitis B among healthcare professionals in sub-Saharan Africa has been linked to inadequate use of protective barriers, needle stick injuries, and insufficient immunization coverage (Coppola et al., 2022). In a study conducted in Kenya, a high prevalence of splash and percutaneous injuries among HCWs was attributed to failure in adhering to universal precautions (Mbaisi et al., 2023). Similarly, in Ethiopia, over one-third of HCWs reported occupational exposure to body fluids due to poor compliance with standard precautions (Tariku et al., 2023).

Needlestick and sharp object injuries, which are common forms of occupational hazards, continue to occur with alarming frequency. These injuries not only result in direct harm but also pose a long-term psychological burden on the affected workers due to fear of infection transmission (Elseviers et al., 2024). Studies in various settings have shown that inadequate response to such injuries, including failure to report and seek medical evaluation, is often associated with lack of knowledge and institutional support (Sharma et al., 2022; Karadag, 2022).

Central Hospital Oleh, a secondary healthcare facility in Delta State, serves a diverse and growing population. The facility, like many others in Nigeria, experiences challenges related to infection control, including shortages in PPE, irregular training, and lack of monitoring systems to enforce adherence to standard precautions. These structural and systemic issues often result in inconsistent application of infection prevention guidelines and place both patients and HCWs at increased risk (Okechukwu & Motshedisi, 2022).

Previous studies conducted in different parts of Nigeria have revealed knowledge gaps and poor practices of standard precautions among HCWs. For instance, a study conducted in Enugu State found that while most health workers had some awareness of standard precautions, only a small proportion consistently practiced them, largely due to institutional and infrastructural limitations (Nwoga, Ajuba, & Ogugua, 2019). Another study conducted in Uyo revealed that HCWs in private facilities demonstrated higher compliance with standard precautions than their counterparts in public hospitals, mainly due to better supervision and resource availability (Johnson et al., 2022).

Furthermore, evidence suggests that even when healthcare workers are knowledgeable about standard precautions, their actual practices may not align with that knowledge. This disparity may be due to cognitive dissonance, workload pressure, or institutional barriers that discourage compliance (Eman et al., 2022). In addition, attitudes toward infection control protocols play a crucial role in influencing behavior. A positive attitude and institutional culture that prioritize patient and staff safety are often associated with better compliance (Agu et al., 2023).

It is also important to consider the role of training and continuing education in strengthening the implementation of standard precautions. Studies have shown that periodic training significantly improves both knowledge and practice among healthcare workers (Gordon & Afayo, 2024). Unfortunately, in many Nigerian healthcare institutions, training is either irregular or absent altogether, leading to inconsistent practices and increased vulnerability to infections (Mehtar, 2021).

Given the rapid emergence and re-emergence of infectious diseases—including the COVID-19 pandemic—there is a renewed urgency to strengthen infection prevention and control practices across all levels of healthcare. The pandemic exposed weaknesses in global health systems and underscored the need for robust, well-enforced infection control measures to protect healthcare providers and patients alike (Sydnor & Perl, 2021).

In light of these realities, assessing the knowledge and practice of standard precautions among healthcare workers in Central Hospital Oleh is both timely and essential. Such an assessment will provide empirical data needed to understand existing gaps, evaluate current practices, and develop targeted interventions to improve compliance. It will also support hospital management and policymakers in designing programs that enhance patient and staff safety.

Ultimately, improving knowledge and reinforcing consistent practice of standard precautions among healthcare workers will lead to a reduction in healthcare-associated infections, enhanced quality of care, and a safer working environment. The findings from this study are expected to contribute to a better understanding of the infection control landscape in Central Hospital Oleh and serve as a basis for institutional improvement and policy formulation.

1.2 Statement of the Problem

Healthcare workers (HCWs) are constantly exposed to occupational risks such as bloodborne pathogens, needle stick injuries, and body fluid splashes during the delivery of healthcare services. Despite the global emphasis on standard precautions as an essential strategy to reduce healthcare-associated infections (HAIs), poor compliance among HCWs remains a persistent challenge, particularly in developing countries like Nigeria (Ogoina et al., 2023). These lapses have led to increased occupational infections, contributing to the burden of disease transmission within healthcare settings.

Several studies have investigated the knowledge and practice of standard precautions among healthcare workers in Nigeria and other low-resource environments. For instance, Nwoga et al. (2019) and Arinze-Onyia et al. (2022) identified varying levels of awareness and practice gaps in tertiary healthcare institutions. However, most of these studies focus on large urban hospitals, often neglecting smaller secondary healthcare facilities such as Central Hospital Oleh, which serve as critical access points for rural and semi-urban populations.

Furthermore, many previous works tend to assess knowledge or practices in isolation without exploring the interplay between the two. As observed by Agu et al. (2023), HCWs may possess theoretical knowledge but fail to translate it into consistent practice due to systemic barriers such as inadequate resources or poor institutional enforcement. Studies like those by Johnson et al. (2022) and Reda et al. (2022) also revealed that categories of healthcare workers and levels of training affect compliance, but such variables are rarely analyzed together in a single institutional context.

Another notable gap is the limited evaluation of infrastructural and organizational factors that influence compliance. For example, studies often do not assess the availability of personal protective equipment (PPE), ongoing infection control training, or management support—all of which play significant roles in standard precaution adherence (Nwoga, Ajuba, & Onwasigwe, 2023; Gordon & Afayo, 2024). Similarly, the extent to which institutional culture influences compliance behavior remains underexplored.

Thus, there is a clear need for a comprehensive study that not only assesses knowledge and practice but also examines contextual factors and the relationship between these variables. This study aims to fill that gap by investigating the knowledge and practices of standard precautions among healthcare workers in Central Hospital Oleh, thereby providing evidence-based recommendations for policy and institutional improvement.

1.3 Objectives of the Study

The main objective of this study is to assess the knowledge and practices of standard precautions among healthcare workers in Central Hospital Oleh, Delta State.

The specific objectives are:

  1. To assess the level of knowledge of healthcare workers on standard precautions.
  2. To examine the extent to which healthcare workers in Central Hospital Oleh practice standard precautions.
  3. To determine the relationship between healthcare workers’ knowledge and their practice of standard precautions.

1.4 Research Questions

The study will be guided by the following research questions:

  1. What is the level of knowledge of standard precautions among healthcare workers in Central Hospital Oleh?
  2. To what extent do healthcare workers in Central Hospital Oleh practice standard precautions?
  3. Is there a relationship between healthcare workers’ knowledge of standard precautions and their actual practice?

1.5 Research Hypotheses

The following hypotheses will be tested in this study:

  1. There is no significant difference between the knowledge of standard precautions and the years of experience of healthcare workers.
  2. There is no significant relationship between the level of knowledge of standard precautions and the actual practices among healthcare workers.
  3. There is no significant association between the category of healthcare worker (e.g., nurses, doctors, lab technicians) and their practice of standard precautions.

1.6 Significance of the Study

The outcomes of this study are expected to be highly beneficial to a broad range of stakeholders in the healthcare sector. One of the primary beneficiaries will be healthcare workers, who are at the forefront of patient care and are directly exposed to occupational hazards. By assessing their knowledge and practices of standard precautions, the study will raise awareness about the importance of consistent adherence to infection prevention guidelines. It will help them recognize existing gaps in their understanding and daily practices, thereby encouraging self-improvement and increased vigilance in their routine duties.

In addition to individual health workers, the study will offer significant insights to hospital management. Administrators and supervisors at Central Hospital Oleh will be able to use the findings to identify areas where their staff may require additional support or training. This will enable the implementation of more effective and targeted in-service training programs tailored to address specific deficiencies in knowledge or practice. Furthermore, it will help management allocate resources more efficiently, ensuring that essential items such as gloves, masks, disinfectants, and waste disposal materials are adequately supplied and accessible to staff at all times.

Policy makers and public health authorities stand to gain from the evidence that this study will generate. By highlighting the realities and challenges faced by healthcare workers in a secondary healthcare facility, the study will inform the development of practical, context-specific policies on infection prevention and control. These policies can be integrated into broader health strategies at the state and national levels, ensuring that guidelines are not only theoretically sound but also feasible for implementation across various levels of the healthcare system.

Researchers and academics will also benefit from this study as it contributes to the growing body of literature on infection control practices in Nigeria. The study’s findings will serve as a reference point for future investigations, especially those aiming to explore the relationship between knowledge, attitude, and practice in different healthcare contexts. It will also highlight the specific needs of secondary healthcare facilities, which are often overlooked in favor of larger tertiary institutions during research and policy formulation.

1.7 Scope of the Study

This study is focused on Central Hospital Oleh, located in Delta State. It will cover all categories of healthcare workers in the hospital, including doctors, nurses, laboratory scientists, and other clinical staff involved in patient care. The study will assess both knowledge and practices related to standard precautions, with a particular emphasis on hand hygiene, use of personal protective equipment, safe injection practices, and waste disposal. Non-clinical staff and administrative personnel will be excluded.

1.8 Operational Definition of Key Terms

Standard Precautions: A set of infection control practices used to prevent transmission of diseases that can be acquired by contact with blood, body fluids, non-intact skin, and mucous membranes.

Healthcare Workers (HCWs): All professionals involved in direct patient care, including doctors, nurses, laboratory scientists, and other allied health personnel.

Knowledge: The awareness and understanding that healthcare workers have about standard precautions, including their principles, benefits, and application.

Practice: The actual implementation of standard precautionary measures by healthcare workers during the delivery of healthcare services.
