Abstract

This research investigated the frequency and intensity of depressive symptoms connected to hyperglycemia and hypertension among inhabitants of Abbi Community in Delta State, Nigeria. The study adopted a quantitative survey research design, utilizing a sample of 20 participants chosen via simple random sampling. Information was gathered through structured questionnaires and examined using descriptive statistical methods, including frequency distributions, means, and standard deviations. The results showed that hyperglycemia and hypertension were common among the participants, with mean blood sugar levels at 157.65 mg/dL and average blood pressure readings at 142.15 mmHg. The investigation further found that depressive symptoms occurred among those diagnosed with hyperglycemia and hypertension, although the association between depressive symptoms and these health conditions was weak and negative. Nevertheless, socio-demographic variables such as age, gender, educational attainment, and occupation were observed to affect the frequency and intensity of depressive symptoms. The outcomes indicate that while hyperglycemia and hypertension can contribute to depressive symptoms, other socio-demographic elements may also exert a substantial influence. The study concludes that managing depressive symptoms in people with hyperglycemia and hypertension demands an integrated strategy that accounts for both medical and socio-demographic considerations. In light of these results, healthcare professionals in Abbi Community are advised to adopt combined health programs that address both physical and mental well-being. Furthermore, public health education initiatives should be launched to raise awareness about the connection between long-term health issues and mental health. Additional studies involving a bigger sample size and incorporating more variables are recommended to improve the applicability of the results.

CHAPTER ONE

INTRODUCTION

1.1 Background to the Study

Depression is a widespread mental health condition that substantially impacts people’s general well-being and quality of life. It involves ongoing feelings of sadness, diminished interest in everyday activities, and difficulties with cognitive functions. Worldwide, depression ranks as a primary cause of disability, producing major effects on social, economic, and health-related results (Czeisler et al., 2020). In recent times, greater attention has been given to the connection between physical health problems and mental health disorders. Among the physical conditions associated with depression, hyperglycemia and elevated blood pressure emerge as significant factors.

Hyperglycemia, which refers to unusually high blood glucose levels, represents a frequent metabolic issue, particularly in people with diabetes mellitus. It may result in complications including neuropathy, nephropathy, and retinopathy (Wu et al., 2022). In the same way, hypertension (high blood pressure) constitutes a key cardiovascular risk element that increases the likelihood of stroke, heart attack, and chronic kidney disease (Hajjar & Kotchen, 2023). The simultaneous presence of hyperglycemia and hypertension, commonly seen in metabolic syndrome, can heighten the chance of serious depressive symptoms (Nabi et al., 2021).

Recent investigations have shown that hyperglycemia and hypertension not only raise the danger of physical health problems but also play a role in triggering and worsening depressive symptoms. Ongoing stress, inadequate blood sugar regulation, and extended hypertension can damage brain functioning and encourage neuroinflammation, thereby causing mood-related issues (Sandström et al., 2022). For example, individuals with hypertension show a higher tendency to experience psychiatric conditions such as depression and anxiety, which in turn make the handling of their physical health issues more difficult (Axon, Zhao, & Egede, 2022).

Even though there is an expanding collection of studies exploring the relationship between depression and metabolic disorders, only a few have concentrated on rural areas, especially in Nigeria. The National Health and Nutrition Examination Survey (NHANES) has played a key role in determining the occurrence and risk elements tied to hypertension and depressive symptoms in the United States (CDC, n.d.; Patel et al., 2019). Still, conclusions drawn from high-income nations might not apply completely to settings with limited resources, where access to healthcare is restricted and cultural views on mental health vary considerably (Ryan, Trosclair, & Gfroerer, 2022).


In Abbi Community, Delta State, where financial difficulties and restricted healthcare services are common. The frequency and intensity of depressive symptoms in people with hyperglycemia and hypertension have not been thoroughly studied. Available research suggests that persons suffering from ongoing illnesses like hypertension and diabetes face greater vulnerability to developing depression (Ong et al., 2021; Giltay, Zitman, & Kromhout, 2024). Declining mental health not only diminishes individuals’ quality of life but also obstructs proper control of their physical conditions, thereby raising the possibility of further complications (Crim et al., 2022).

Additionally, depression has been linked to overall mortality and deaths from ischemic heart disease in adults who have hypertension, positioning it as an important public health issue (Axon, Zhao, & Egede, 2022). The interaction between mental and physical health underscores the necessity for a complete healthcare strategy, particularly in communities with limited services.

Comprehending the overlap between these conditions is vital for creating unified healthcare programs. Consequently, this study seeks to evaluate the frequency and intensity of depressive symptoms related to hyperglycemia and high blood pressure in Abbi Community. This examination will offer important understanding of the mental health challenges encountered by individuals with metabolic disorders in this rural environment, thus helping to shape policies and programs designed to enhance health results.

1.2 Statement of the Problem

Although a strong link between depression, hyperglycemia, and hypertension has been clearly established across different global populations (Axon et al., 2022; Nabi et al., 2021), a clear deficiency exists in the available literature concerning this connection in rural Nigerian communities, notably Abbi Community in Delta State. Earlier studies have mainly targeted urban groups that enjoy superior healthcare access and resources (Czeisler et al., 2020; Sandström et al., 2022). In addition, the majority of research has aimed at identifying broad associations instead of specifically investigating the intensity and occurrence of depressive symptoms in connection with hyperglycemia and high blood pressure.

Moreover, past research has frequently overlooked cultural, socio-economic, and healthcare access factors that could shape depressive symptoms in rural locations (Rantanen et al., 2018; Yang et al., 2018). There is also inadequate proof regarding the combined effects of these conditions on mental health within a rural Nigerian context. Although works such as those by Ong et al. (2021) and Cutler et al. (2022) have delivered useful information on hypertension awareness and management, they do not tackle the possible mental health effects of these conditions in disadvantaged communities.

Therefore, the issue this study intends to tackle is the shortage of concrete empirical information on the frequency and intensity of depressive symptoms linked to hyperglycemia and hypertension in Abbi Community. This research aims to bridge this gap by supplying data that can form the foundation for creating practical, culturally appropriate interventions suited to the community’s particular requirements.

1.3 Objectives of the Study

The primary goal of this study is to evaluate the frequency and intensity of depressive symptoms connected to hyperglycemia and hypertension in Abbi Community, Delta State, Nigeria. The specific objectives are:

1. To establish the prevalence of depressive symptoms among individuals diagnosed with hyperglycemia and hypertension in Abbi Community.

2. To investigate the severity of depressive symptoms linked to hyperglycemia and hypertension within the affected population.

3. To determine the socio-demographic factors that affect the frequency and intensity of depressive symptoms among individuals with hyperglycemia and hypertension.

1.4 Research Questions

The following research questions will direct the study:

1. What is the prevalence of depressive symptoms among individuals diagnosed with hyperglycemia and hypertension in Abbi Community?

2. What is the severity of depressive symptoms associated with hyperglycemia and hypertension among residents of Abbi Community?

3. What socio-demographic factors influence the prevalence and severity of depressive symptoms among individuals with hyperglycemia and hypertension?

1.5 Research Hypotheses

The study is guided by these hypotheses:

1. H₀: There is no significant prevalence of depressive symptoms among individuals diagnosed with hyperglycemia and hypertension in Abbi Community.

H₁: There is a significant prevalence of depressive symptoms among individuals diagnosed with hyperglycemia and hypertension in Abbi Community.

2. H₀: Hyperglycemia and hypertension do not significantly influence the severity of depressive symptoms among residents of Abbi Community.

H₁: Hyperglycemia and hypertension significantly influence the severity of depressive symptoms among residents of Abbi Community.

3. H₀: Socio-demographic factors do not significantly affect the prevalence and severity of depressive symptoms among individuals with hyperglycemia and hypertension.

1.6 Significance of the Study

This study holds importance for multiple reasons. Its findings will expand the current knowledge base regarding the intricate relationship between physical and mental health. By analyzing the association between hyperglycemia, hypertension, and depressive symptoms in a rural Nigerian setting—specifically Abbi Community in Delta State—the research delivers useful insights into how persistent health conditions can affect mental health outcomes in areas with limited resources. The study’s results can assist policymakers and healthcare providers in recognizing the value of incorporating mental health services into the standard treatment of chronic illnesses, especially hyperglycemia and hypertension. This is critical for formulating more complete healthcare policies that tackle mental health requirements together with physical health issues, particularly in rural environments with scarce resources.

By pinpointing the frequency and intensity of depressive symptoms related to hyperglycemia and hypertension, this investigation will enable community health workers and caregivers to develop and apply focused mental health programs. These programs can greatly enhance the quality of life for those affected and support the general welfare of the community. The study will function as a core reference point for future investigations into the links between long-term physical ailments and mental health disorders in rural settings. It will also point out possible directions for additional exploration and motivate more researchers to examine the overlap between physical and mental health in comparable environments.

1.7 Scope of the Study

This research will take place in Abbi Community, situated in Delta State, Nigeria. The study population will include individuals diagnosed with hyperglycemia and hypertension who live in the community. The focus will be on evaluating the prevalence and severity of depressive symptoms associated with these conditions. Data collection will occur via structured questionnaires, with analysis performed using statistical methods to identify correlations and causal links.

Depressive Symptoms: Emotional and behavioral signs of depression, such as ongoing sadness, reduced interest, fatigue, and cognitive difficulties.
